Use
The 1p19q Deletion by FISH and IDH1 R132H Point Mutation by Immunohistochemistry with Reflex to IDH1 and IDH2 Mutation Analysis, Exon 4 test is a preferred initial test for diagnosing oligodendrogliomas. It detects IDH1 or IDH2 mutations and the co-deletion of 1p/19q, both of which are critical for a definitive diagnosis of oligodendrogliomas. This test is vital for distinguishing these tumors from other types of gliomas and can guide treatment decisions, such as the use of chemotherapy regimens.
Special Instructions
This test requires tumor tissue to be formalin-fixed and paraffin-embedded. A tissue transport kit is available for transporting the block or slides. If multiple specimens are submitted, instructions should be included to allow the ARUP pathologist to select the most appropriate specimen for testing, otherwise, submissions are held until clarification. Negative IDH1 IHC results will reflex to IDH1 and IDH2 Mutation Detection to check for other possible mutations.
Limitations
The test cannot detect variants outside the targeted genomic regions. It does not cover copy number alterations, translocations, RNA variants, deep intronic variants, or small insertions/deletions beyond its detection limits. Variants in low quality regions may not be detected, and the test cannot distinguish between somatic and germline variants.

Collection Instructions
Formalin fix (10% neutral buffered formalin is preferred) and paraffin embed specimen. Transport tissue block or 10 unstained slides, positively charged in a tissue transport kit.
Storage Instructions
Room temperature or refrigerated. Ship in a cooled container during summer.
Causes for Rejection
Paraffin block with less than 25% tumor tissue. Specimens fixed in other fixatives or decalcified specimens.
