C4D by Immunohistochemistry

Use
This test is a laboratory-developed immunohistochemistry (IHC) stain service offered by ARUP Laboratories. It detects complement split product C4d deposition in formalin-fixed, paraffin-embedded tissue or cellblock preparations. C4d deposition in capillaries is clinically used as an indicator of antibody-mediated rejection in organ transplant pathology, particularly in kidney and heart allografts.
Special Instructions
Specimens must be formalin fixed (10% neutral buffered formalin) and paraffin embedded (cells into cellblock). Provide either tissue blocks or at least 5 unstained, positively charged slides (3–5 µm thick), minimum 2 slides, in ARUP transport kit (supply #47808). Do not oven-bake precut slides. Specimens should be protected from excessive heat. Submit electronically or via ARUP IHC Stain Form (#32978) if needed. This is a stain-and-return (technical) service with no interpretation included.
Limitations
Frozen, non‑formalin‑fixed, or non‑paraffin‑embedded specimens are unacceptable. Sections must be prepared appropriately; unsuitable specimens include non‑representative tissue type or depleted specimens. Frozen specimens are unacceptable. Stability is indefinite at ambient or refrigerated temperatures; frozen specimens are not acceptable. Interpretation requires a separate consult if pathologist insight is desired.
