Use
This test is a laboratory-developed immunohistochemistry (IHC) stain service offered by ARUP Laboratories. It detects complement split product C4d deposition in formalin-fixed, paraffin-embedded tissue or cellblock preparations. C4d deposition in capillaries is clinically used as an indicator of antibody-mediated rejection in organ transplant pathology, particularly in kidney and heart allografts.

Limitations
Frozen, non‑formalin‑fixed, or non‑paraffin‑embedded specimens are unacceptable. Sections must be prepared appropriately; unsuitable specimens include non‑representative tissue type or depleted specimens. Frozen specimens are unacceptable. Stability is indefinite at ambient or refrigerated temperatures; frozen specimens are not acceptable. Interpretation requires a separate consult if pathologist insight is desired.
