Use
The Comprehensive Muscle Biopsy Workup is used for the detailed examination of muscle tissue to diagnose various neuromuscular disorders. By analyzing the muscle biopsy using a series of detailed microscopic, immunohistochemical, and electron microscopic techniques, this test assists in diagnosing diseases like inclusion body myositis, limb-girdle muscular dystrophy, myofibrillar myopathies, and others. Clinical correlation is required for accurate interpretation of the results, and it is crucial for understanding the etiology of neuromuscular symptoms.
Special Instructions
Prior to the collection of the muscle biopsy, obtain the Muscle/Nerve Biopsy collection kit (ARUP supply #40923) through ARUP Connect™ or by contacting Client Services. It is important to follow the provided collection instructions closely to prevent drying of specimens and ensure optimal biopsy preparation. Electronic requests are preferred, but if unavailable, use an ARUP Anatomic Pathology Form (#32960) with an ARUP client number.
Limitations
Results from the Comprehensive Muscle Biopsy Workup should be interpreted with caution, as the presence of mild inflammatory infiltrates and certain muscle fiber necroses may not definitively confirm specific diseases such as inclusion body myositis. The presence of TDP-43 positive inclusions and mitochondrial abnormalities may suggest certain myopathies but are not solely diagnostic. There is also a potential for suboptimal preparation if the collection kit instructions are not properly followed, possibly leading to delayed or inaccurate results.

Collection Instructions
Obtain Muscle/Nerve Biopsy collection kit prior to collection including special fixatives. Ensure fresh specimens are kept moist with saline gauze and fixed specimens are submerged in fixative.
Storage Instructions
Store at room temperature. Refrigerate if necessary and ship in cooled container during summer months.
Causes for Rejection
Non-representative tissue type or failure to adhere to collection instructions.
