Leukotriene E4, 24-Hour Urine

Use
This test aids in the initial diagnosis of suspected mast cell activation disorders. It measures the concentration of leukotriene E4 (LTE4) in a 24-hour urine collection. High levels of LTE4 may indicate conditions such as systemic mastocytosis or other mast cell activation disorders. Monitoring LTE4 levels can be useful in assessing the effectiveness of treatment or progression of the disorder.
Special Instructions
Patients should discontinue 5-lipoxygenase inhibitor zileuton (Zyflo) 48 hours prior to urine collection if possible, as it may decrease LTE4 levels. The total collection volume of urine must be greater than 300 mL for the test to be valid. Collection duration and urine volume must be accurately recorded and provided for testing.
Limitations
Test performance may be affected by the use of 5-lipoxygenase inhibitor zileuton (Zyflo) which may decrease LTE4 concentrations. The test is not FDA cleared or approved. It is essential to collect a complete 24-hour urine to ensure accuracy of results. Incomplete collection or loss of urine during the collection period may affect test results.

Collection Instructions
Collect a 24-hour urine sample, refrigerate during collection, and ensure that the total volume is accurately measured and recorded. Transfer 5 mL from a well-mixed 24-hour collection to ARUP standard transport tubes.
Patient Preparation
Discontinue zileuton (Zyflo) 48 hours prior to sample collection, if possible.
Storage Instructions
Store the collected urine sample frozen. It is also acceptable to store it refrigerated.
Causes for Rejection
Total collection volume less than 300 mL, incomplete collection, and failure to provide collection duration and volume.
