Use
The presence of elevated and persistent antiphosphatidylserine (aPS) and prothrombin (PT) antibodies, specifically IgG and IgM, with or without lupus anticoagulant activity, may act as a risk marker for thrombosis in patients with certain autoimmune diseases such as antiphospholipid syndrome (APS) and systemic lupus erythematosus (SLE). This test may be useful when other antiphospholipid (aPL) antibody tests are negative, helping to document persistence of antibodies associated with these conditions.
Special Instructions
After blood collection, it is crucial to separate serum from cells as soon as possible or within 2 hours. Additionally, always transfer the specimen (either serum or citrate plasma) to an ARUP standard transport tube to ensure proper handling. Appropriate storage conditions include refrigeration, with ambient and frozen as secondary options.
Limitations
The test is limited in its ability to distinguish between autoimmune disease types without clinical correlation. It may not detect all variations of anticardiolipin antibodies unless persistently elevated and supported with clinical signs of thrombotic disorders. Moreover, false positives could occur due to other coagulopathies or medication interference, emphasizing the need for comprehensive patient evaluation and history.

Collection Instructions
Collect using a serum separator tube (SST) or a light blue sodium citrate tube. Ensure prompt separation from cells either immediately or within 2 hours of collection.

Causes for Rejection
Contaminated, heat-inactivated, clots, fibrin, gross red blood cells, severely lipemic, severely hemolyzed, or severely icteric specimens.
