Use
AP3B2 (adaptor protein 3 beta2)-IgG is a marker of an autoimmune disorder characterized by gait instability as the predominant neurologic presentation. Patients present with cerebellar, dorsal column, or sensory neuronal dysfunction. The test is useful for the differential diagnosis of mixed cerebellar and sensory ataxia and myeloneuropathy. Clinical improvement following treatment has been reported. AP3B2 autoimmunity appears rare and potentially treatable.

Limitations
A negative result does not exclude neurological autoimmunity or cancer. The test's performance characteristics were developed by Mayo Clinic in a manner consistent with CLIA requirements. It is not cleared or approved by the US FDA, and improper specimen collection, handling, and inappropriate test selection can lead to diagnostic confusion.
