Use
This test is used for guiding dosage adjustments of busulfan to achieve complete bone marrow ablation while minimizing dose-dependent toxicity before hematopoietic stem cell transplantation for chronic myelogenous leukemia. It measures the area under the curve (AUC) and clearance of busulfan to facilitate dose adjustment, ensuring adequacy of dosing and avoiding potential toxicity, such as veno-occlusive liver disease, seizures, and coma.
Special Instructions
The test requires specific specimen collection times following the first dose of intravenous busulfan. Four plasma specimens must be collected under one order: immediately after infusion, and 1, 2, and 4 hours post-infusion. Specimens must be refrigerated or kept on wet ice and centrifuged within 2 hours of collection. Physicians must submit infusion start/completion times, medication dose, body weight, and age with specimens. A completed Busulfan Information: Mail-In Specimen Instructions form is mandatory for processing the test.
Limitations
The AUC results are dependent on the correct collection and handling of the plasma specimens as described. Incorrect protocol execution may lead to inaccurate pharmacokinetic evaluations and potentially misguided dosing adjustments. Doe limit valid for specific dosing protocols (0.8 mg/kg every 6 hours) and may not apply to alternative dosing regimens or specimens collected outside the prescribed timeline. Gross hemolysis, lipemia, or icterus do not invalidate the test results.

Collection Instructions
Collect 4 plasma samples: one immediately post first intravenous busulfan infusion, and at 1, 2, and 4 hours post-infusion. Keep on wet ice or refrigerated, centrifuge within 2 hours, then separate plasma and store in labeled plastic vials. Freeze immediately at -20°C.
