Use
This test is used for testing cortisol levels in the right adrenal vein. It is particularly useful as a second-order test when cortisol measurements by immunoassay do not align with clinical symptoms, or if there is a suspicion of exogenous synthetic steroid use. The test aids in the differential diagnosis of primary and secondary adrenal insufficiency and Cushing syndrome.
Special Instructions
For confirming the presence of synthetic steroids, users are recommended to order SGSS / Synthetic Glucocorticoid Screen, Serum. When ordering, ensure to include the time of specimen collection and send separate orders for multiple specimens.
Limitations
Cortisol assays should use the same methodology throughout for serial monitoring. An isolated p.m. serum cortisol measurement holds little diagnostic value. Increased plasma cortisol levels in women can be caused by high circulating estrogen levels, which do not affect the free, bioactive cortisol fraction. Acute stress, certain medications, and improper handling can affect cortisol levels and interfere with test results, masking the signs of congenital adrenal hyperplasia and complicating Cushing syndrome diagnosis.
