Use
This test is used for the evaluation of known or suspected hematologic neoplasms, specifically of myeloid origin, such as acute myeloid leukemia, myelodysplastic syndrome, and myeloproliferative neoplasms. It aids in determining the diagnostic classification, provides prognostic or therapeutic information to guide clinical management, and evaluates for new clinically significant gene mutation changes at relapse. It is also useful for evaluating patients with suspected VEXAS syndrome.
Special Instructions
The following information is required with the specimen: clinical diagnosis, pertinent clinical history including disease phase and therapy status, clinical suspicion, date of collection, and specimen source. Bone marrow and whole blood specimens must arrive within 14 days of collection.
Limitations
The test covers 47 genes but will not detect genetic abnormalities outside of these target areas. It detects single base substitutions and small insertions or deletions but does not identify gene rearrangements, fusions, copy number alterations, or large deletions. The assay does not differentiate between somatic and germline alterations for certain variant allele frequencies and may detect mutations not clearly distinguishable from tumor-associated mutations in older individuals. Results can be affected by prior treatment or stem cell transplantation, requiring correlation with clinical, histopathologic, and additional laboratory findings.

Invert several times to mix bone marrow. Send bone marrow specimen in original tube. Do not aliquot. Label specimen as bone marrow.
Causes for Rejection
Gross hemolysis, bone marrow biopsies, slides, paraffin shavings or frozen tissues, paraffin-embedded tissues, paraffin-embedded bone marrow aspirates, moderately to severely clotted.
