Use
This test is designed for evaluating multiple myeloma at the time of diagnosis and at disease relapse. It provides prognostic information and determines potential therapeutic implications. Next-generation sequencing (NGS) technology enables detailed evaluation of multiple myeloma genetics, allowing for further risk categorization through the identification of genetic abnormalities that have prognostic and potential therapeutic value. This test includes next-generation sequencing of 26 genes: BIRC3, BRAF, CCND1, CDKN2A, CRBN, CUL4A, CUL4B, CXCR4, DIS3, EGFR, IDH1, IDH2, IKZF1, IKZF3, KRAS, MYC, MYD88, NRAS, NSD2, PIK3CA, PIM1, STAT3, TENT5C, TP53, TRAF3, and XBP1.
Special Instructions
Bone marrow aspirate samples must arrive within 4 days of collection. Submit either bone marrow aspirate, paraffin-embedded bone marrow clot, or bone marrow clot slides with a minimum plasma cell percentage specified in the specimen requirements. Fresh specimens are required as testing is performed on sorted cells.
Limitations
This NGS panel is targeted and will not detect genetic abnormalities outside the specified 26 genes. It detects single base substitutions and small insertion/deletion type events but is not configured to detect gene fusions, large-scale deletions, or structural genomic rearrangements. The test does not differentiate between somatic and germline alterations. Inadequate samples or prior treatment effects may impact results. NGS should not be pursued if the initial plasma cell percentage is below 5%.
