Mitochondrial Antibodies (M2), Serum
Use
This test is used for establishing the diagnosis of primary biliary cholangitis (PBC), a chronic autoimmune liver disease characterized by the progressive destruction of intrahepatic bile ducts, leading to liver damage. Positivity of antimitochondrial antibodies (AMA), particularly the M2 type, is a serological hallmark of PBC, occurring in 90% to 95% of patients. These autoantibodies mainly target the E2 subunits of several mitochondrial enzyme complexes, playing a critical role in evaluating the disease.

Limitations
This test is not useful for indicating the stage or prognosis of primary biliary cholangitis or for monitoring the disease's course. Positive results may infrequently be found in patients with systemic sclerosis, other autoimmune diseases, and relatives of patients with PBC. Testing for PBC-specific ANA antibodies may be necessary for patients who are AMA-negative but are suspected of having PBC.
