Measurement of total protein for the assessment of orthostatic proteinuria; orthostatic proteinuria refers to protein excretion that increases when upright and resolves when supine—typically seen in children, adolescents, or young adults; this test evaluates whether protein excretion remains elevated when supine, supporting or excluding orthostatic proteinuria, often performed on three consecutive mornings for robustness.

Limitations
False proteinuria may result from contamination with menstrual blood, prostatic secretions, or semen; newborns may have higher urinary protein excretion during the first 3 days of life; presence of hemoglobin may elevate protein concentration; further protein characterization may require protein electrophoresis or immunofixation.

Collection Instructions
Nighttime (supine) collection: void immediately prior to lying down; collect an 8‑hour (nighttime) urine specimen; upon awaking, patient should immediately void into specimen cup; invert well before taking 4‑mL (maximum) aliquot; no preservative.
Patient Preparation
Specimens should be collected before fluorescein is given or not collected until at least 24 hours later.
Causes for Rejection
All specimens will be evaluated at Mayo Clinic Laboratories for test suitability.
