Voriconazole, Serum
Use
Voriconazole (Vfend) is an antifungal agent approved for treatment of invasive aspergillosis and candidemia/candidiasis, as well as for salvage therapy for infections in patients refractory to, or intolerant of, other antifungal therapy. Monitoring trough levels of voriconazole is suggested for individuals with reduced liver function, those with CYP2C19 alterations associated with poor metabolic function, patients taking other medications affecting CYP2C19 activity, and those experiencing potential toxicity. It assists in ensuring adequate oral absorption and improving treatment response.
Special Instructions
Trough level monitoring is important as high levels above 6 mcg/mL have been associated with toxicity, whereas levels below 1 mcg/mL indicate a suboptimal response. Within 2 hours of collection, centrifuge, and aliquot serum into a plastic vial. Ensure compliance with collection container protocols.
Limitations
The metabolism of voriconazole can be significantly altered by the coadministration of drugs that induce or inhibit cytochrome P450 2C19. Genetic alterations affecting enzyme activity may also alter metabolism, impacting the test results. The test has not been cleared or approved by the FDA.
