Use
CD123 serves as a diagnostic, prognostic, and therapeutic marker in some hematologic malignancies, notably acute leukemia. Its co-expression with TCF4 by IHC is highly specific and sensitive for blastic plasmacytoid dendritic cell neoplasm. CD123 positive acute myeloid leukemia (AML) is associated with high marrow blast percentages, monocytic differentiation, and CD34 negativity, suggesting its role in maintaining the leukemic state. The test is technical only, excluding a pathologist read.
Special Instructions
Specimens like fresh tissues should be fixed in neutral buffered formalin for a specific period before potential transfer to 70% ethanol for longer-term storage. Slides need to be on positively charged glass to avoid tissue loss and background staining, and they should be unbaked.

Collection Instructions
Fixative should be at a 20:1 ratio with tissue, using 10% neutral buffered formalin for 6-72 hours (48-72 hours preferred).
Storage Instructions
Store at 15°C to 25°C. Use a refrigerated gel pack in shipment to protect from extreme temperatures, separating the gel pack from the specimen.
Causes for Rejection
Specimens not fixed in 10% neutral buffered formalin or B plus will be rejected.
