Formalin‐fixed, paraffin‐embedded (FFPE) tissue block is preferred OR one unbaked, unstained slide for H&E staining (required) plus two to three (2‑3) positively charged unstained slides (4‑5 microns) per test/antibody ordered. Block and slide identifiers must match specimen ID exactly.
