Preferred: formalin‑fixed, paraffin‑embedded (FFPE) tissue block. Alternatively: one unbaked, unstained H&E slide (required) plus two to three positively charged unstained slides (4‑5 microns) per test/antibody ordered; block and slide identifiers must match requisition.
