Elevated alpha‑fetoprotein (AFP) levels in pleural fluid may indicate malignancy, including hepatocellular carcinoma and other cancers (e.g., ovarian, gastrointestinal, pulmonary), but can also be seen in benign hepatic disease. Interpretation should consider clinical context and corroborate with other diagnostic studies.
