Use
IgG antibodies to 3-hydroxy-3-methylglutaryl-coenzyme A reductase (HMGCR) are mainly associated with necrotizing autoimmune myopathy (NAM) in a subset of statin-treated patients. These antibodies can aid in the differential diagnosis of myositis in patients with or without statin exposure. They may also be occasionally observed in statin-naive patients with NAM. Strong clinical correlation is recommended in the absence of muscle fiber necrosis, elevated serum creatine kinase, perimysial pathology, and/or statin exposure.

Limitations
Interpretation of the test should be done in the context of the patient's clinical presentation. The presence of IgG antibodies to HMGCR does not definitively indicate necrotizing autoimmune myopathy (NAM), nor does their absence rule it out. There may be other causes of muscle pathology that this test does not account for.
