Special Instructions

Blood should be drawn for baseline androstenedione before administering cosyntropin. After injection of cosyntropin (250 μg IM or IV, diluted in 2-5 mL of saline for IV), a second blood sample should be collected 60 minutes later. Use of red-top tubes is required, and specimens need to be transported together with accurate labeling, including patient information and collection details.

Limitations

The test may not provide accurate results if specimens are not handled properly, such as using gel-barrier tubes, which can cause significant absorption and affect results. Factors such as time of day and recent glucocorticoid administration can influence androgen levels and result interpretation. Thus, sample consistency regarding these parameters is crucial for accurate assessments.
