Use
This test is useful for diagnosing primary membranous nephropathy (PMN) and distinguishing it from secondary nephrotic syndrome. The semiquantitative measurement of PLA2R antibody levels by ELISA is suggested for monitoring response over time in individuals undergoing treatment for membranous nephropathy. A positive PLA2R antibody result, in conjunction with clinical symptoms and other laboratory findings, is suggestive of primary membranous nephropathy.
Special Instructions
Ensure to separate serum from cells as soon as possible, or within 2 hours of collection. Transfer 1 mL serum to an ARUP standard transport tube. Maintain specimens under refrigerated conditions for transport.
Limitations
A positive anti-PLA2R antibody result suggests primary membranous nephropathy but the absence of circulating antibodies does not rule out the diagnosis. Antibody titers are useful for assessing disease severity and monitoring clinical remission; low antibody titers are associated with disease remission, while high titers indicate loss of kidney function and may necessitate a more aggressive therapeutic approach.
