Clinical Use

Use

This panel is used for evaluating patients with suspected autoimmune liver disease, specifically conditions like autoimmune hepatitis or primary biliary cholangitis. It helps in the diagnostic workup of liver disease of unknown etiology and assesses the presence of specific liver-related autoantibodies. The panel includes testing for antinuclear antibodies (ANA), anti-mitochondrial antibodies (AMA), and smooth muscle antibodies (SMA), which are significant in diagnosing autoimmune hepatitis and primary biliary cholangitis.

Special Instructions

This test requires proper specimen handling. Serum should be centrifuged and aliquoted into a plastic vial after collection. For order submissions outside electronic systems, a Gastroenterology and Hepatology Test Request form should accompany the specimen. The test should be used for evaluating patients at risk for antinuclear antibody-associated systemic autoimmune rheumatic disease.

Limitations

The presence of smooth muscle antibodies (SMA), antinuclear antibodies (ANA), and antimitochondrial antibodies (AMA) should not be solely relied on for diagnosing autoimmune liver diseases. Positive results can occur in other conditions such as drug exposure-related hepatitis or CREST syndrome. Autoantibodies are not specific and should be interpreted in the context of clinical presentation and liver disease parameters. The test does not indicate the stage or prognosis of the disease nor is it suited for monitoring disease progression.
