Bupropion and Metabolite, Serum or Plasma

Use
Bupropion is an antidepressant drug indicated for the treatment of major depressive disorder and is also used for smoking cessation. It's predominantly metabolized to hydroxybupropion, which retains about 50% of the parent drug's activity. This test measures the serum levels of bupropion and its metabolite to optimize drug therapy and monitor patient adherence. The therapeutic monitoring is essential due to the influence of drug-drug interactions on CYP2B6 metabolism and the need for dose adjustments in patients with renal or hepatic impairment.
Special Instructions
This test should be ordered to optimize drug therapy and monitor patient adherence with regard to bupropion use. The timing of specimen collection should be a predose (trough) draw at steady state concentration. The specimen should be separated from cells ASAP or within 2 hours of collection and transferred to an ARUP Standard Transport Tube for accurate results.
Limitations
The pharmacokinetics of bupropion and its metabolite can be affected by drug-drug interactions that influence CYP2B6 metabolism, potentially requiring dose adjustments. The assay may not fully account for all interacting factors affecting drug levels and activity. Adverse effects such as seizures, hypertension, and neuropsychiatric issues may occur.
