Use
This test is used to optimize drug therapy and monitor patient adherence by measuring carbamazepine and its active metabolite, Carbamazepine-10,11-Epoxide. It provides relevant information for therapeutic drug monitoring, particularly for patients at risk of adverse drug reactions such as Stevens-Johnson syndrome which is associated with the presence of the HLA-B*15:02 allele in certain populations.
Special Instructions
Patient preparation involves a pre-dose (trough) draw at steady-state concentration. It is critical to collect the specimen in plain red, lavender (K2 or K3EDTA), or pink (K2EDTA) tubes and separate serum or plasma from cells within 2 hours of collection.
Limitations
The therapeutic range for Carbamazepine-10,11 Epoxide is not well established. The test may not cover all potential genotypes related to carbamazepine hypersensitivity, indicating the necessity for additional pharmacogenetic testing when appropriate.
