Use

The Celiac Disease Comprehensive Cascade is used for evaluating patients suspected of having celiac disease. This test includes an algorithmic evaluation encompassing human leukocyte antigen (HLA) typing. It helps in identifying individuals with a high probability of celiac disease, which can assist in deciding the need for invasive procedures like a small intestinal biopsy. The test checks for various serologic markers that can increase the probability of diagnosing celiac disease. It is especially useful for individuals with family members who have celiac disease and may have genetic susceptibility due to specific HLA markers.

Special Instructions

This test should not be used for patients who have been or are currently on a gluten-free diet. It is not suitable for individuals who have already had HLA DQ2/DQ8 typing performed. Different cascades are recommended based on patient scenarios, including those on a gluten-free diet or those who only need serology testing.

Limitations

This cascade should not be solely relied upon to establish a diagnosis of celiac disease. It is used to identify patients with an increased probability of having celiac disease for whom a small intestinal biopsy is recommended. HLA typing identifies genetic predisposition but does not confirm the disease. Negative serologic results do not exclude celiac disease, especially in patients who have started a gluten-free diet.
