Use
This test is used to detect recurrent common chromosome abnormalities associated with myeloid/lymphoid neoplasms with eosinophilia and gene rearrangements, including PDGFRA, PDGFRB, FGFR1, JAK2, and ABL1. These rearrangements are crucial for accurately categorizing the disease and can inform potential therapeutic interventions, such as treatment with targeted tyrosine kinase inhibitors. The FISH test also evaluates specimens in which chromosome studies are unsuccessful and serves as an adjunct to conventional chromosome studies in patients with eosinophilia.
Special Instructions
The test requires the specification of the FISH probes to be analyzed in the order request. Missing probe information can lead to test processing delays or cancellations. Bone marrow is the preferred specimen type, but blood can be used if neoplastic cells are confirmed. Shipping instructions emphasize the need for Express Mail if not using a courier service. Necessary information such as a list of requested probes and a reason for testing must accompany the specimen.
Limitations
The test is not FDA-approved and is best used as an adjunct to clinical and pathologic information. It is not a substitute for conventional chromosome studies, which can detect other chromosomal abnormalities that FISH cannot. Bone marrow is the preferred sample; blood used needs to have neoplastic cells present. In the absence of FISH signals post-hybridization, results do not confirm the absence of a neoplastic disorder. Interpretation of results may be compromised by the failure to provide necessary probe and reason for testing information.

Collection Instructions
It is preferable to send the first aspirate from the bone marrow collection. Invert several times to mix bone marrow. Send bone marrow specimen in original tube. Do not aliquot.
