Use

This test is used to monitor disease in individuals diagnosed with various pemphigus variants who have increased IgG desmoglein 1 and/or IgG desmoglein 3 antibodies. The antibody levels correlate with disease activity. ELISA testing for IgG desmoglein 1 and IgG desmoglein 3 antibodies is highly sensitive, with more than 90 percent of patients with IgG-variant pemphigus showing increased levels of one or both antibodies. Specific reactivity to the type of desmoglein helps in determining pemphigus subtypes; increased IgG desmoglein 1 antibody level is associated with pemphigus foliaceus, while increased IgG desmoglein 3 antibody level is associated with pemphigus vulgaris, clinically presenting with mucosal and skin lesions.

Special Instructions

For initial diagnosis, the Pemphigus Antibody Panel, IgG (0090650) is preferred due to greater diagnostic sensitivity and specificity. Consider Immunobullous Disease Antibody Panel (3001409) for a comprehensive evaluation. For rare types of pemphigus, consider Pemphigus Antibodies, IgA by IIF (0092106) and Paraneoplastic Pemphigus Expanded Antibody Panel by IIF with ELISA (3016534).

Limitations

The ELISA results for IgG desmoglein 1 and 3 antibodies do not rule out pemphigus foliaceus or vulgaris, evolving or treated pemphigus, non-desmoglein antibody pemphigus, or other immunobullous diseases. Patients with cell surface antibody-positive pemphigus by indirect immunofluorescence can have normal ELISA results due to antibodies targeting different desmoglein 1 and/or 3 epitopes than those displayed. Diagnosis and monitoring should consider additional testing and clinical correlation with symptoms and treatment status.
