Immunobullous Disease Antibody Panel
Use
The Immunobullous Disease Antibody Panel is used for initial comprehensive testing to aid in the diagnosis and differentiation of skin and mucosal disorders that present with blistering, eczema, erosions, pruritus, and/or urticaria. It assesses suspected epithelial antibody-associated immunobullous diseases such as pemphigoid, pemphigus, and their variants that are not clinically distinguishable. The panel is especially useful when concurrent Direct Immunofluorescence with Tissue Biopsy indicates such disorders, or in cases due as potential side effects of immune checkpoint inhibitor (anti-PD-1, anti-PD-L1) therapy.
Special Instructions
When ordering, consider companion tests for more specific suspected conditions such as Celiac Disease Reflexive Cascade or Epidermal Transglutaminase (eTG/TG3) Antibody, IgA by ELISA if dermatitis herpetiformis is a concern. The test requires serum and should be transported quickly to ensure stability.
Limitations
Negative results for IgG, IgG4, and IgA basement membrane zone antibodies do not rule out diseases such as bullous pemphigoid, epidermolysis bullosa acquisita, and linear IgA bullous dermatosis, as antibodies may not be detectable in all cases. Additionally, negative ELISA results for antibodies like BP180, BP230, and type VII collagen may miss some cases due to variability in disease presentation and antibody expression.

Collection Instructions
Collect in a plain red or serum separator tube (SST), then transfer 2 mL serum to a transport tube.
Storage Instructions
Specimens should be stored refrigerated unless shipping promptly (within 2 hours). Transport at ambient temperature to arrive within 7 days. If storage is 7-14 days until received, store and ship refrigerated. Store and ship frozen if more than 14 days.
Causes for Rejection
Hemolyzed, lipemic specimens, and plasma are unacceptable.
