A recent American Congress of Obstetricians and Gynecologists (ACOG) practice bulletin made recommendations as to the appropriate use of laboratory tests that should be performed as part of the investigation of the cause of late term (greater than 20 weeks) fetal demise.6 The authors suggested that maternal testing for thyroid stimulating hormone (TSH), human parvovirus B19 IgG and IgM, lupus anticoagulants, and anticardiolipin antibodies could provide useful information for future pregnancy management.

Special Instructions

This test requires careful specimen handling, including citrate plasma collection by double centrifugation and adherence to specific anticoagulant protocols. A discard tube is necessary when using butterfly collection devices. Specimen transport must avoid delays, especially for frozen samples, and patient must avoid certain therapies before testing.

Limitations

Testing schedules may vary, and additional confirmatory or reflex tests might extend turnaround times. The validity of certain detectors is compromised in patients on anticoagulant therapies, potentially leading to false positives. Reliable testing mandates strict adherence to specified blood collection and handling protocols.
