Use
This test is useful for the evaluation of kidney disease using a 24-hour urine collection. It also serves as a screening tool for monoclonal gammopathy. Protein in urine can provide insights into defects in glomerular filtration, tubular reabsorption, or increased plasma protein concentration.
Special Instructions
The 24-hour urine volume in milliliters must be provided when sending the specimen. Collection instructions state that urine should be collected for a full 24 hours, and ensure no preservative is used. It is important to wait 24 hours post fluorescein administration before specimen collection, and to avoid overfilling the aliquot tube.
Limitations
The test may give false results if the urine is contaminated with menstrual blood, prostatic secretions, or semen. Additionally, normal proteinuria levels can be elevated post-exercise and protein excretion may be higher in newborns. Any total protein measurement exceeding 500 mg/24 hours should lead to further evaluation by immunofixation to identify monoclonal immunoglobulin light chains.
