Use
This test is useful for the diagnosis and management of patients with renal lithiasis. It helps in predicting the likely composition of the stone in patients who have a radiopaque stone but lack available stone analysis. The test can aid in designing a treatment program, identify specific risk factors for stones using a 24-hour urine collection, and monitor the effectiveness of therapy by confirming decreased crystallization potential. It evaluates kidney excretion of acid, urine pH, and estimates a patient's protein intake.
Special Instructions
The addition of preservative or application of temperature controls must occur at the beginning of the collection. If not ordering electronically, complete, print, and send a Renal Diagnostics Test Request with the specimen.
Limitations
The test was developed and validated in compliance with CLIA requirements but has not been cleared or approved by the FDA. Improper specimen collection and handling, inappropriate test selection, and interfering substances can cause diagnostic confusion. Treatment may reduce the supersaturation with respect to one crystal type but increase supersaturation with another.

Collection Instructions
Add 5 mL of diazolidinyl urea as preservative at start of collection or refrigerate specimen during and after collection. Collect urine for 24 hours and ensure pH is between 4.5 and 8. Specimens with pH above 8 indicate bacterial contamination and testing will be canceled.
Patient Preparation
X-ray dyes and contrast media will affect uric acid results. If a kidney X-ray with dye or CT scan with contrast has been performed, wait a minimum of 1 day before starting collection. For cholangiography, wait 7 days before collection.
Storage Instructions
Refrigerated is preferred for up to 14 days, ambient for up to 72 hours, or frozen for up to 14 days.
Causes for Rejection
Specimens with pH above 8 indicate bacterial contamination and testing will be canceled. Do not attempt to adjust pH.
