This test aids in confirming the presence of synthetic glucocorticoids in serum specimens, especially in the context of suspected glucocorticoid exposure or when evaluating the cause of glucocorticoid-induced adrenal insufficiency. These synthetic glucocorticoids are extensively used as anti-inflammatory and immunosuppressive agents, and the test is crucial in detecting synthetic glucocorticoids that might not be documented in medical records, such as those used in non-prescription preparations. The test can also assess exposure leading to Cushing syndrome features or adrenal insufficiency through HPA axis suppression.

Special Instructions

This test is not useful for the detection of fluticasone propionate. Collection involves centrifuging and aliquoting serum into a plastic vial. Ensure proper specimen handling to avoid diagnostic confusion or interference from external substances.

Limitations

The assay is limited to confirming only a specified list of synthetic glucocorticoids and cannot detect all synthetic steroids available. Detection may not account for previous exposures due to the long-term effects from discontinued glucocorticoids and their clearance rates, which depend on administration route and glucocorticoid type. Additionally, not all routes of glucocorticoid exposure might be captured due to their influence on metabolism.
