TMAO is a metabolite produced by the gut microbiome after consumption of dietary precursors such as L‑carnitine, choline, and phosphatidylcholine. Elevated TMAO levels are associated with increased risk of atherosclerosis, enhanced platelet reactivity, thrombosis, and major adverse cardiovascular events, including a roughly 2.5-fold increased risk of incident MACE, with meta-analyses showing a 7.6% relative increase in CVD mortality per 10 µmol/L increment.
