Use
Envoplakin Antibody, IgG by ELISA is used to aid in the diagnosis of paraneoplastic pemphigus (PNP), also known as paraneoplastic autoimmune multiorgan syndrome (PAMS), a rare disease associated with lymphoproliferative disorders and malignancies presenting with severe pemphigus-like clinical features and a high mortality rate. This test helps in assessing disease extent and activity, particularly in monitoring cases with increased IgG envoplakin antibody levels, making it useful in conjunction with other clinical indicators and histopathological assessments.

Limitations
An increased or normal IgG envoplakin antibody level by ELISA, as a stand-alone test, cannot completely confirm or rule out PNP/PAMS; it requires correlation with other tests and clinical features. The test has a high specificity of 96-99% and variably reported sensitivity of 55-86%. ELISA may be more sensitive than indirect immunofluorescence for low-level antibodies but might not detect the broad range of epithelial targets possible with immunofluorescence. False negatives could occur if only this ELISA is relied on without using complementary testing methods.
