Motor and Sensory Neuropathy Evaluation with Immunofixation Electrophoresis and Reflex to Titer and Neuronal Immunoblot

Use
This test aids in diagnosing combined motor/sensory neuropathy, particularly when a plasma cell dyscrasia or other malignancy is suspected. The evaluation includes detection of antibodies through several immunoassays, which are associated with various peripheral neuropathies. For example, anti-GM1 antibodies are linked with motor or sensorimotor neuropathies, while GD1b antibodies are related to sensory ataxic neuropathy syndrome.
Special Instructions
Purkinje cell (PCCA) antibody and neuronal nuclear (ANNA) antibody IgG are screened by IFA. If the IFA screen is indeterminate, a Neuronal Nuclear Antibodies (Hu, Ri, Yo, and Tr/DNER) IgG by Immunoblot test is added. If the IFA screen is positive at 1:10 or greater, a PCCA/ANNA antibodies titer and additional tests are added, incurring extra charges.
Limitations
The presence of ganglioside antibodies is not solely diagnostic and should be interpreted in conjunction with other clinical parameters. These tests are not FDA cleared or approved, but are performed in a CLIA certified laboratory. Factors such as contamination, heat-inactivation, and severe hemolysis can affect the test results.
