Special Instructions
The major metabolite, norclomipramine, retains biologic activity. Specimens for therapeutic monitoring should be collected immediately before the next dose. It is crucial not to use gel-barrier tubes due to potential drug absorption by the gel.
Limitations
The LC/MS-MS methodology was developed and its performance characteristics determined by LabCorp, but it is not cleared or approved by the FDA. Metabolism of clomipramine may be dose-related and subject to enzyme inhibition from medications like valproic acid, which could lead to elevated clomipramine levels and potential seizures.
