11-Deoxycortisol, Serum
Use
This test is primarily useful for the diagnostic workup of patients with congenital adrenal hyperplasia, assessing suspected secondary or tertiary adrenal insufficiency, and is part of the metyrapone testing in the differential diagnostic workup of Cushing syndrome. 11-Deoxycortisol levels provide insights into the hypothalamic-pituitary-adrenal axis function by evaluating cortisol precursor levels.
Special Instructions
The specimen should ideally be collected in the morning (8 a.m.) and is part of the metyrapone testing procedure. It's important to indicate whether the specimen was collected before or after metyrapone administration.
Limitations
Ethical considerations around specimen collection and the metyrapone test application include the potential for impaired response due to ethanol, estrogens, barbiturates, valproic acid, phenytoin, and glucocorticoids. Special care is needed during testing to avoid an Addisonian crisis, especially during the 2-day metyrapone regimen, which is generally avoided due to logistical challenges.
