11‑Desoxycortisol
Use
Assists in the diagnostic workup of congenital adrenal hyperplasia and in metyrapone testing for distinguishing forms of adrenal insufficiency or Cushing syndrome. The test reflects levels of 11‑deoxycortisol, an immediate cortisol precursor, which typically rises markedly after metyrapone; impaired response may indicate hypoadrenalism or hypopituitarism.
Special Instructions
NY State approved test; performed by Esoterix Endocrinology at Mayo Clinic Laboratories. Aliases include 11‑Deoxycortisol and Compound S. Report available in 6 to 15 days after specimen receipt.
Limitations
Interference possible from exogenous substances including ethanol, estrogens (exogenous or pregnancy-related), barbiturates, valproic acid, phenytoin, and exogenous glucocorticoids; metyrapone testing may precipitate Addisonian crisis in susceptible individuals.

Collection Instructions
Draw blood in a plain red‑top tube; serum gel tube acceptable. Separate serum from cells within 45 minutes. Spin down and send 1 mL of serum frozen in plastic vial.
