Amikacin, Trough, Serum
Use
Amikacin is an aminoglycoside used to treat severe infections caused by susceptible strains of gram-negative bacteria. It works by irreversibly binding to bacterial ribosomes, thereby inhibiting protein synthesis and inducing cell death. The test is useful for monitoring the adequate clearance of amikacin near the end of a dosing cycle, ensuring effective treatment while preventing toxicity.
Special Instructions
If not ordering electronically, complete, print, and send a Therapeutics Test Request (T831) with the specimen. Ensure serum gel tubes are centrifuged and serum is aliquoted within two hours of collection. This test requires careful coordination with the laboratory for proper specimen collection and handling.
Limitations
Serum half-life will be prolonged, and significant drug accumulation will occur in patients with impaired renal function due to excretion primarily by glomerular filtration. Timing of sample collection is critical for accurate interpretation. Potential toxic effects include auditory and nephrotoxicity, occurring particularly at higher concentrations, which necessitates regular monitoring.
