Clinical Use

Use

This test is useful for screening and confirmation of drug abuse or use of buprenorphine. It begins with a screening immunoassay; if positive, a confirmation by liquid chromatography–tandem mass spectrometry is performed. The immunoassay has a screening cutoff of 5 ng/mL for buprenorphine and confirmation is triggered for buprenorphine ≥ 5 ng/mL or norbuprenorphine ≥ 2.5 ng/mL, supporting detection of recent use. It helps monitor compliance, detect misuse, or confirm therapeutic exposure consistent with substitution therapy or analgesic use.

Special Instructions

For chain-of-custody testing, order BUPMX / Buprenorphine and Norbuprenorphine, Chain of Custody, Random, Urine. If adulteration or urine creatinine is required, order the ADULT / Adulterants Survey, Random, Urine. No preservative should be used. STAT requests are not accepted. A Therapeutics Test Request (T831) requisition is required if ordering non-electronically.

Limitations

Immunoassay screens can produce false-positive or false-negative results due to cross-reactivity with unrelated substances or interfering compounds. Results may be influenced by factors such as fluid intake or specimen integrity. Submitting less than the required volume may compromise testing. Interpretation should consider possible interferences, and confirmation by LC‑MS/MS is required for positive screens.
