CNS Demyelinating Disease Evaluation, Serum
Use
This test is useful for diagnosing inflammatory demyelinating diseases (IDDs) that have a similar phenotype to neuromyelitis optica spectrum disorder (NMOSD), such as optic neuritis and transverse myelitis. It discriminates NMOSD, acute disseminated encephalomyelitis (ADEM), and other conditions from multiple sclerosis early in the disease course. It aids in diagnosing ADEM, autoimmune myelin oligodendrocyte glycoprotein-opathy, and neuromyelitis optica, predicting a relapsing disease course, and guiding appropriate treatment strategies to prevent symptom exacerbation, especially in cases with a distinct diagnosis from multiple sclerosis.
Special Instructions
For optimal antibody detection, specimen collection is recommended before starting immunosuppressant medication. This test is available for NY State clients. Multiple neurological phenotype-specific autoimmune/paraneoplastic evaluations are available; refer to the Autoimmune Neurology Test Ordering Guide for more testing options.
Limitations
Aquaporin-4-IgG and myelin oligodendrocyte glycoprotein-IgG antibodies might drop below detectable levels during acute attack therapies or attack prevention treatments. The test may not eliminate all possibilities of false results if antibodies fall below detection thresholds due to treatment effects.
