Contactin-1 IgG Cell Binding Assay, Serum
Use
The Contactin-1 IgG Cell Binding Assay is used primarily for evaluating chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) and related demyelinating peripheral neuropathies. It aids in determining contactin-1 IgG results as part of a profile. CIDP is an acquired, immune-mediated condition affecting peripheral nerves and nerve roots, leading to significant disability. Patients with antibodies targeting nodal-paranodal proteins, such as the neurofascin-155-contactin-1 complex, show common immunopathological mechanisms, distinct clinical features, and treatment responses.
Special Instructions
For optimal antibody detection, specimen collection is advised before starting immunosuppressant medication or intravenous immunoglobulin treatment (IVIg). The test is only orderable as part of a larger profile, specifically the CIDP/Nodopathy Evaluation and the Peripheral Nervous System Demyelinating Neuropathy, Autoimmune Evaluation. Specimen handling requires centrifuging and aliquoting serum into a plastic vial.
Limitations
A negative result does not rule out an immune-mediated demyelinating disease. The test should be used within the appropriate clinical context, and the administration of immunotherapy prior to sample collection may negatively impact the sensitivity of these assays. Interpretation of results should be made in consideration of the overall clinical presentation.
