Use
The test is used for second-order testing when cortisol measurement by immunoassay gives inconsistent results with clinical symptoms, or if patients are taking synthetic steroids. It serves as an adjunct in diagnosing primary and secondary adrenal insufficiency as well as Cushing syndrome. It is not recommended for evaluating response to metyrapone.
Special Instructions
Preferred morning (8 a.m.) and afternoon (4 p.m.) specimens should be submitted. Provide the time of collection and ensure samples are properly centrifuged and aliquoted into a plastic vial.
Limitations
This test is not recommended for evaluating response to metyrapone. Conditions such as stress, alcoholism, depression, and medication can affect the diurnal variation. High estrogen levels in women can falsely elevate plasma cortisol levels without increasing bioactive cortisol.
