Cryptococcus Antigen with Reflex, Spinal Fluid
Use
This test aids in the diagnosis of cryptococcosis, an invasive fungal infection predominantly caused by Cryptococcus neoformans or Cryptococcus gattii. The infection is usually acquired through the pulmonary route, but it can also involve the central nervous system, especially in immunocompromised patients. Cryptococcal infections can be severe, with a high mortality rate if untreated or if the central nervous system is involved. It is important for determining antifungal therapy needs and possible dissemination of the disease.
Special Instructions
This test should not be used as a test of cure or to guide treatment decisions. It is not recommended for screening the general population and should only be performed when clinical evidence suggests the diagnosis of cryptococcal disease. If ordering electronically, complete the Infectious Disease Serology Test Request form and send it with the specimen.
Limitations
A negative result does not rule out the diagnosis of cryptococcosis, especially if only a single specimen has been tested and the patient exhibits symptoms consistent with the disease. False-negative results can occur with extremely high concentrations of cryptococcal antigen due to weak test lines. A positive result is indicative of cryptococcosis, but all results should be interpreted in conjunction with other clinical findings. The assay has not been evaluated for cross-reactivity in patients with trichosporonosis.
