Use
This test is useful for monitoring whole blood peak cyclosporine concentration during therapy, especially for individuals taking cytochrome P450 (CYP) 3A4 substrates, inhibitors, or inducers. It aids in adjusting doses to optimize immunosuppression while minimizing toxicity and evaluating patient compliance.
Special Instructions
This test should be used for specimens collected during a peak period. For specimens collected at trough, a different test (CYCSP / Cyclosporine, Blood) should be ordered. Necessary information includes the date and time of the last dose, as well as dosage information.
Limitations
No definitive therapeutic or toxic ranges have been established for peak testing. Therapeutic drug monitoring is essential due to cyclelosporine's narrow therapeutic range and interactions with other drugs leading to increased neuro- and nephrotoxicity. Results of this assay, measured by liquid chromatography with tandem mass spectrometry, are approximately 30% less than those from immunoassay.
