Dihydrorhodamine Flow Cytometric Phorbol Myristate Acetate Test, Blood
Use
This test is useful for evaluating chronic granulomatous disease (CGD), including both X-linked and autosomal recessive forms, and complete myeloperoxidase deficiency. It monitors chimerism and nicotinamide adenine dinucleotide phosphate oxidase (NOX) function post-hematopoietic cell transplantation, assesses residual NOX activity pre-transplant, identifies female carriers for X-linked CGD, and evaluates changes in lyonization with age in female carriers.
Special Instructions
Testing is not performed on Saturday, Sunday, or observed holidays. Specimens must be collected and shipped to arrive when testing is performed. Specimens received outside of stability on non-testing days will be canceled. The ordering healthcare professional's name and phone number are required.
Limitations
Specimens should be collected in sodium heparin and optimally tested within 24 hours of blood draw (stability is within 48 hours). The test may be normal or mildly impaired in patients deficient in NCF4 (p40phox). Severe glucose-6-phosphate dehydrogenase deficiency can mimic CGD and affect DHR responses. Hemolyzed specimens may interfere with the assay. Specimens with an absolute neutrophil count less than 200 will be rejected. Complete myeloperoxidase deficiency may yield false-positive results requiring differentiation by specific fluorescence patterns and recombinant human MPO addition tests.
