Dilute Russell's Viper Venom Time (DRVVT) Confirmation, Plasma
Use
The Dilute Russell's Viper Venom Time (DRVVT) Confirmation test is useful for confirming the presence or absence of lupus anticoagulants (LA). It helps to identify LA that do not prolong the activated partial thromboplastin time (APTT) and evaluates unexplained prolongation of the APTT or prothrombin time clotting tests. This test is also used to distinguish between LA and specific coagulation factor inhibitors or deficiencies.
Special Instructions
The DRVVT confirmation is performed only when the DRVVT screen is abnormally prolonged. It is not orderable as a standalone test but as part of reflexive or additional testing scenarios. Consider other coagulation-based tests in conjunction for comprehensive assessment of antiphospholipid antibodies.
Limitations
Lupus anticoagulants are heterogeneous, and no single coagulation test can identify or exclude all LA. Residual platelets in frozen-thawed plasma can decrease the test's sensitivity and specificity, causing false-negative results. Anticoagulation therapy can result in false positives; thus, repeat testing remote from therapy is suggested. The DRVVT will not detect all LAs, as some may only be detectable by other methods.
