Use
This test is useful in aiding the differentiation between primary and secondary polycythemia. It helps in distinguishing between appropriate secondary polycythemia (caused by conditions such as high-altitude living, pulmonary disease, or tobacco use) and inappropriate secondary polycythemia (such as those caused by tumors). It is also useful in identifying candidates for erythropoietin (EPO) replacement therapy, such as those with chronic kidney disease, and evaluating patients undergoing EPO therapy who exhibit an inadequate hematopoietic response.
Special Instructions
Morning collection is preferred due to diurnal variation to ensure consistency. If not ordering electronically, complete and submit one of the designated request forms along with the specimen.
Limitations
Erythropoietin (EPO) levels alone cannot reliably distinguish between primary and secondary polycythemia, as EPO levels may be within normal limits in some patients with primary polycythemia. Assays may differ significantly among commercial EPO tests, and it's recommended to use the same test for serial assessments. The presence of heterophile antibodies can interfere with this assay, and discrepancies in results should prompt further investigation. The assay cannot distinguish between endogenous and exogenous EPO, nor can it measure recombinant EPO compounds accurately; their levels can be estimated roughly based on assay cross-reactivity.
