Extended-Spectrum Beta-Lactamase-Producing Gram-Negative Bacteria Surveillance Culture, Feces
Use
This test is useful for screening colonization of extended-spectrum beta-lactamase (ESBL)-producing organisms in stool. It is also beneficial for screening fecal microbiota transplant donor feces for ESBL-producing organisms. ESBLs are enzymes that confer resistance to certain cephalosporins and aztreonam, raising infection control issues due to their transmissibility. The test involves culture techniques to isolate ESBL-producing Enterobacterales, with confirmatory testing using disk diffusion methods.
Special Instructions
The test includes reflex testing where additional charges may apply based on initial test results. It is crucial for all bacterial isolates to be tested to rule out ESBL production, particularly targeting species such as Escherichia coli, Klebsiella pneumoniae, Klebsiella oxytoca, and Proteus mirabilis. Specimens must be shipped within 96 hours of collection.
Limitations
Not all ESBL-producing organisms may be detected by this method. The screening is not intended for medicolegal use. Improper specimen collection and handling or inappropriate test selection can affect results. Only those organisms cultivatable under standard laboratory conditions will be detected. Results are based on the utilization of selective media and associated confirmatory testing methods.

Collection Instructions
Collect fresh fecal specimen and place 1 gram or 5 mL in container with transport medium. Place feces in preservative within 2 hours of collection and place vial in a sealed plastic bag.
