Fentanyl with Metabolite Confirmation, Random, Urine
Use
This test is useful for the detection and confirmation of illicit drug use involving fentanyl. Fentanyl is an extremely fast-acting synthetic opioid that interacts predominantly with the opioid mu-receptor. It is metabolized primarily to norfentanyl and other inactive metabolites. The presence of fentanyl above 0.20 ng/mL or norfentanyl above 1.0 ng/mL is a strong indicator that the patient has used fentanyl.
Special Instructions
For situations requiring chain of custody, a Chain of Custody Kit (T282) is available. STAT requests are not accepted for this procedure. If urine creatinine is required or adulteration is suspected, order ADULT / Adulterants Survey, Random, Urine. Submitting less than 5 mL will compromise the ability to perform all necessary testing.
Limitations
Urine concentrations do not correlate well to serum drug levels, and very high concentrations of butyl fentanyl and acetyl fentanyl can interfere with the test, causing a low positive fentanyl concentration. Proper specimen collection and handling are crucial.
