Glycine Receptor Alpha1 IgG, Cell Binding Assay, Serum
Use
Evaluating patients with suspected autoimmune stiff-person spectrum disorders, including stiff-person syndrome, stiff-limb and stiff trunk syndromes, or progressive encephalomyelitis with rigidity and myoclonus (PERM), using serum specimens. These disorders are associated with exaggerated startle, stiffness, and spasms of the axis and/or limbs. The test is particularly useful for diagnosing patients who are seronegative for GAD65-IgG but may have antibodies against the GlyRa1 antigen.
Special Instructions
When not ordering electronically, a Neurology Specialty Testing Client Test Request form (T732) should be completed, printed, and sent with the specimen.
Limitations
Negative results do not exclude autoimmune stiff-person spectrum or cancer. The antibody is detected in 19% of patients who are GAD65-IgG seronegative. Disease-specific antibodies may be present in serum, CSF, or both. False negatives can occur if the antibody concentration is below the detection limit.
